Be Well Primary Health Care Center, LLC is an Article 28 Diagnostic and Treatment Center that is proudly serving the Brooklyn, NY community since 2004. Our first location in Flatbush has long been recognized as a trusted, one-stop-shop health care facility offering comprehensive, evidence-based care to a diverse community of patients. Expanding our reach and continuing our commitment to accessible, high-quality care, we opened a second location in Bedford-Stuyvesant, further extending our reach to serve more of Brooklyn’s diverse population.

Across both of our state-of-the-art facilities, patients benefit from a full spectrum of services, from preventive and routine primary care to a wide range of specialties including dentistry, optometry, ophthalmology, physical therapy, podiatry, and more. Our approach goes beyond simply treating illness. We are intentional about creating a care environment that is welcoming, efficient, and designed to drive long-term health outcomes. Every decision is guided by our commitment to proactive, patient-centered care.

We accept most insurances and are currently welcoming new patients of all ages and backgrounds. Appointments can be scheduled by calling or texting us at 718-253-WELL. You can also send us a message and our staff will reach out to help you book.
